Participant Information

Thank you for your interest in having your youth participate in Freewheelin’s classes. In order for us to provide high quality programming, we ask that a representative of a youth (a parent or relative, guardian, sponsoring agency) complete this form. Not only will it help us serve your youth but it will help us track our performance and plan for future programming. Dates for each class can be found in the Events Calendar.

We do ask that a small fee be paid for each youth’s participation in a program. It serves as an “investment” in your youth’s participation in our program. Please plan to pay when you have been informed that your youth has been added to the next class’ roster. The first day of class is acceptable. The fee is payable by cash, check or credit card to “Freewheelin’ Community Bikes.”

If the registration fee is a barrier, it can be paid in increments or it can be paid at the end of the program. The fee can be waived if it represents a true hardship. Because this fee does not begin to cover the total costs of our programming, AND if you can afford to pay more, please consider making a donation to Freewheelin’ Community Bikes.

This form should be completed by the parent or guardian of the youth being registered.  The information on the form is about the youth/participant, unless the caption mentions parent or guardian.

Registration fee is $25.  This is part of your “investment” in your son or daughter’s participation in our program. If the registration fee is a barrier to your child attending, you may pay as you go, wait until the end of the program or ask the program coordinator for the fee to be waived. The most important part is that your child participate fully in all of the classes. AND if you can afford to pay more, please consider making a donation to Freewheelin Community Bikes.

Please plan to pay when you have been informed that your youth has been added to the next class’ roster.  The first day of class is acceptable. The fee is payable by cash, check or credit card to “Freewheelin Community Bikes.”

You are invited to attend our first class, last class and the riding days but are by no means required to attend. Most families drop their child off.

The first section of the form below should be filled out with the Youth’s information. If youth does not have email or phone number, use a family member’s email and phone.
